In Ontario, the practice of commissioning is governed by the Commissioners for Taking Affidavits Act. The Act gives Municipal Clerks and other Officials (also known as Commissioners of Oaths) the authority to take various types of declarations and to administer oaths, when you sign an affidavit or statutory declaration.
Affidavits are verified documents containing sworn statements. All parties required to sign the document must be present and must provide valid photo identification in order to have a document commissioned. The Clerk or designate (Commissioner of Oaths) will review the document to determine if it meets the specifications.
Commissioners of Oaths services are offered at the Town of Ingersoll Municipal Office. Since the nature of documents varies greatly, it is strongly recommended that you contact the Municipal Office prior to your visit to confirm that your attestation can be commissioned by one of our staff members.
A charge of $15 will be applied for all documents requiring the signature of a Municipal Commissioner of Oaths.
Please note:
- The Municipality does not have a Notary Public on staff, and the Municipality's Commissioners of Oaths cannot notarize documents or certify true copies.
- The Commissioner of Oaths reserves the right to refuse commissioning services of any document.
- The Municipal Office is not responsible for the content of the affidavit
- Commissioners cannot offer any legal advice, or prepare or edit affidavits/statements. Any assistance pertaining to an affidavit should be directed to the agency or party that has requested the form.
Declarants must sign in the presence of a Commissioner of Oaths. Photo identification must also be presented.

- Domestic and Foreign Pension documents (Proof of Life)

- Statutory Declaration of OSAP – Statement of Common-Law
- Status, Supporting Children
- Statutory Declaration of Apprenticeship Hours
- Statutory Declaration of Lost Identification (must include police report number)

- Affidavit of Unregistered Vehicle (boards, motorcycles, snowmobiles, trailers, etc.)
- Consent Letter for Children Travelling Abroad (as witness)
